  • Practice and Outcomes Manager
    Assists the Hospitalist Site Director, Hospitalists and HMG to establish practice operating procedures to successfully achieve Quality Outcomes and fulfill Hospitalist Service contractual terms. In coordination with the HMP Site Director, evaluates practice needs, establishes Hospitalist Service staffing schedule, conducts meetings and implements procedures to ensure timely, high quality service outcomes to patients and Hospital. This individual manages and ensures appropriate daily office operations and serves as the liaison between corporate office personnel, physicians, patients, hospital personnel and the public in general.

  • Gaylord Hospital is a 137-bed long-term acute care hospital (LTACH) that specializes in managing the complex medical care and rehabilitation of patients with multiple acute health care needs. A patient's average length of stay is 25-30 days. 
  • The medical division consists of the medically complex program for patients who require multidisciplinary medical services after an acute hospital stay, ventilator care program and pulmonary program.  Conditions treated include chronic respiratory failure with ventilator weaning, severe COPD, post operative complications, complicated wounds, post transplant care, complicated pneumonia, pulmonary edema and CHF, sepsis, multi-system/organ failure, and sepsis.
  • The rehabilitation division serves those with spinal cord injury, traumatic brain injury, complicated strokes, neurological diseases and complicated orthopedic cases.
  • Gaylord Hospital is equipped to manage patients' care with four critical care beds, cardiac monitoring, ventilator weaning, bariatric rooms, positive and negative pressure rooms.
  • The hospital also has on-site radiology services with a new CT scan, ultrasound, x-ray, and modified barium swallow, as well as its own laboratory.
  • Gaylord has one of the most comprehensive brain injury rehabilitation programs in the East. Although the hospital has treated people with brain injuries for decades, the formal program began in 1980.

Hospitalist Program Profile

 

  • The Hospitalist team of 8 physicians will be on-site 24 hours per day, 7 days a week.  Each full time hospitalist will provide 168 to 192 hours of clinical professional medical services per month consisting of 12-hour shifts. 
  • The Hospitalist team will complete daily rounds, provide support for the daily care needs of the patient, work with care team to establish and meet goals as they relate to the patient's care plan and to facilitate patient admissions/discharges.
  • The Hospitalist team will provide service response for codes, consultations, inpatient episodic care and procedures for complex medical and pulmonary patients including ventilator weaning.
